Update of the template with (new) thesis requirements (#18):
- The dean has changed; the new name is now included on the title page
- Title page no longer includes country of birth when the place of birth is outside of the Netherlands
- Adds a “Contributions” page for chapters based on multi-author papers
- Add cslreferences environment needed for newer versions of pandoc (@LDSamson, #4)
- First version of package with complete documentation
